    Advent Talent Group is seeking a Designated Manager for our client, a growing Minnesota-based care organization dedicated to delivering high-quality home and community-based services.

    This pivotal leadership role goes beyond compliance—it shapes culture, drives quality, and influences the future of the organization.     Overview:
    Location: Brooklyn Park, Minnesota (On-Site)
    Employment Type: Full-Time, Monday – Friday
    Pay Rate: $31.25/hour
    Position Type: Contract to hire

    Responsibilities
    Compliance & Regulatory Oversight

    • Serve as the Designated Manager per Minnesota Statute 245D.081 with daily oversight of program operations
    • Ensure compliance with 245D, 245A, and related Minnesota rules (service delivery, documentation, staff training, incident reporting)
    • Oversee licensing renewals, variances, audits, and investigations
    • Maintain policies, procedures, and corrective action plans per DHS and MDH requirements
    • Conduct site visits and internal audits to verify compliance and quality standards

    Leadership & Team Development

    • Supervise, mentor, and evaluate Designated Coordinators and program leaders
    • Support recruitment, onboarding, and staff training to ensure 245D compliance
    • Foster a culture of respect, teamwork, accountability, and person-centered care

    Program Management

    • Oversee individual service plans (ISPs) and documentation of services delivered
    • Coordinate communication among interdisciplinary teams, guardians, and case managers
    • Monitor staffing ratios, scheduling, and resource allocation for operational efficiency

    Quality Assurance & Reporting

    • Develop quality assurance processes including review of service delivery records, outcomes, and incidents
    • Submit timely, accurate reports to DHS and oversight bodies
    • Analyze data to identify trends, risks, and improvement opportunities
    • Proactively report concerns and progress to leadership

    Strategic Leadership

    • Align 245D operations with broader business goals through collaboration with company leadership
    • Contribute to strategic planning, growth initiatives, and program expansion
    • Drive organizational goals and report progress proactively
    • Represent the company professionally with counties, licensors, and stakeholders

    What You Bring
    To succeed as a Designated Manager, you must meet Minnesota Rule 245D.081, Subd. 1–2 qualifications, which include:
    Education & Experience:

    • Bachelor’s degree in a human services-related field and at least one year of supervisory experience in programs providing direct support services, or
    • Associate’s degree in a related field and at least two years of supervisory experience in a licensed program

    Additional Skills & Requirements:

    • In-depth knowledge of 245D, 245A, and 245C statutes and compliance standards
    • Strong leadership, communication, and organizational abilities
    • Ability to manage multiple priorities with professionalism and confidentiality
    • Valid driver’s license and willingness to travel within Minnesota
    • Genuine passion for person-centered care and in-home health services

    Preferred Experience:

    • Previous role as a Designated Manager in a 245D-licensed setting
    • Familiarity with Home and Community-Based Services (HCBS), Assisted Living, or Home Care programs
    • Experience using DHS licensing platforms (MN-ITS, PELICAN, etc.)
