Advent Talent Group is seeking a Designated Coordinator for a person-center home care service located in Rush City, MN. The Designated Coordinator oversees and coordinates services for individuals supported through the agency’s 245D program. This role ensures services are compliant, organized, and responsive while promoting independence, safety, and community integration. The DC serves as a key liaison between clients, families, staff, and external partners, and plays a leadership role in maintaining quality and regulatory standards.

Pay: $49,219.62 – $65,000 annually
Location: On-site – Rush City, Minnesota

Responsibilities
- Coordinate day‑to‑day 245D service delivery in compliance with regulatory requirements, service agreements, and agency policies
- Develop, implement, and monitor individualized plans and documentation (ISP, PDP, SPA, IAPP, SMAA, BSP) to ensure person‑centered, compliant services
- Participate in intakes, assessments, 45‑day/60‑day meetings, and ongoing support plan reviews
- Serve as the primary point of contact for clients, families, guardians, case managers, community partners, and internal teams
- Supervise, train, mentor, and coach direct support staff; provide guidance on client‑specific supports, plans, and restrictions
- Oversee documentation and records including progress notes, incident reports, audits, and compliance materials; ensure accuracy and timeliness
- Monitor service quality, identify gaps or risks, implement corrective actions, and escalate safety, medical, behavioral, staffing, or compliance concerns promptly
- Manage assigned caseloads, deadlines, authorizations, renewals, signatures, and follow‑up to ensure continuity of care
- Support quality assurance and administrative efforts while working flexible hours, including remote work when necessary, to meet documentation and compliance expectations
Qualifications
- Meets all Minnesota 245D Designated Coordinator requirements
- Minimum 3 years of Designated Coordinator experience required
- Experience in human services, social services, developmental disabilities, or behavioral health
- Proven staff supervision, coaching, and performance support experience
- Strong knowledge of person‑centered practices and behavioral health principles
- High attention to detail with strong compliance and documentation skills
- Excellent organizational, communication, and multitasking abilities
- Willingness to work flexible hours, including remote work, as needed
